Keep a copy of the completed entry form for your records and enclose a copy in your wine shipment.  If you include credit card information on the form, black it out on the shipping copy.  

We also offer the option of entering online via EnofileOnline. Fees for Entries made on EnofileOnline can be paid online, or payment can be made by mailing a check, or by sending credit card information by mail, email, fax or phone.
